چکیده

INTRODUCTION Sphincter of Oddi dysfunction (SOD) is a benign acalculous obstruction to the flow of biliopancreatic secretions through the sphincter of Oddi (1,2). In the general population, its prevalence is 1.5% (3). It is one of the known causes of post-cholecystectomy syndrome, and is observed in 14%–23% of these patients (4,5). The post-cholecystectomy syndrome can loosely be defined as a persistent or recurrent upper abdominal pain (usually right) following cholecystectomy and may be caused by gastroesophageal reflux, gastroparesis, or visceral hyperalgesia. Alternatively, retained common bile duct stones, which may be seen in 4%–7% of patients following cholecystectomy, or complications of the cholecystectomy including bile duct injuries and resultant bile leaks may be responsible for persistent post-cholecystectomy abdominal pain (6). However, if these causes are excluded and cholecystectomy was performed for only clinical biliary symptoms, SOD is the more likely cause for recurrent or persistent symptomatology. Sphincter of Oddi dysfunction produces acute pancreatitis in the possum.

BACKGROUND Sphincter of Oddi dysfunction has been implicated as a cause of various forms of acute pancreatitis. However, there is no direct evidence to show that sphincter of Oddi dysfunction can cause obstruction of trans-sphincteric flow resulting in acute pancreatitis. AIMS To determine if induced sphincter of Oddi spasm can produce trans-sphincteric obstruction and, in combination with st...

Sphincter of Oddi dysfunction: diagnosis and treatment.

Since its original description by Ruggero Oddi in 1887, the sphincter of Oddi (SO) has been the subject of much study and controversy. Its very existence as a distinct anatomic or physiologic entity has been disputed. Hence, it is not surprising that the clinical syndrome of sphincter of Oddi dysfunction (SOD) and its therapy are controversial areas [1]. Nevertheless, SOD is commonly diagnosed ...

Sphincter of Oddi Dysfunction: Introduction

Sphincter of Oddi dysfunction refers to structural or functional disorders involving the biliary sphincter that may result in impedance of bile and pancreatic juice flow. Up to 20% of patients with continued pain after cholecystectomy and 10–20% of patients with idiopathic recurrent pancreatitis may suffer from sphincter of Oddi dysfunction. This condition is more prevalent among middle-aged wo...

Clinical and scintigraphic assessment of the role of endoscopic sphincterotomy in the treatment of sphincter of Oddi dysfunction.

Postcholecystectomy pain caused by sphincter of Oddi dysfunction remains a difficult condition to treat. Endoscopic sphincterotomy has been recommended for those patients with confirmed sphincter of Oddi motor abnormalities. We have studied sphincter of Oddi dysfunction patients to evaluate the effects of endoscopic sphincterotomy on both clinical symptoms and previously reported scintigraphic ...

Sphincter of Oddi: Understanding Its' Control, Function and Disease

The most common functional disorders of the biliary tract and pancreas relate to the activity of the sphincter of Oddi. The sphincter of Oddi is a small smooth muscle sphincter strategically placed at the junction of the bile duct, pancreatic duct and duodenum. Disorder in its motility is called sphincter of Oddi dysfunction. Clinically this presents either with recurrent abdominal pain or epis...

BILIARY TRACT Outcome of endoscopic sphincterotomy in post cholecystectomy patients with sphincter of Oddi dysfunction as predicted by manometry and quantitative choledochoscintigraphy

Background: Sphincter of Oddi dysfunction is diagnosed at manometry and, after cholecystectomy, non-invasively at quantitative choledochoscintigraphy. Patients may benefit from endoscopic sphincterotomy. Aims: The aim of this study was to assess the usefulness of choledochoscintigraphy compared with manometry in predicting outcome of sphincterotomy in post cholecystectomy patients with sphincte...
